know that patients are responding to Provenge. Only in a minority of patients is there a PSA drop or a change in an imaging test, such as a bone scan. The FDA no longer bases its evaluation of response in clinical trials for men with HRPC on a change in PSA, but rather on the extension of life. This FDA evaluation change makes a great deal of sense due to the fact that therapies may not change PSA, but may still extend life. This is what was demonstrated in the Provenge clinical trials. Provenge extended survival in patients with HRPC significantly more than the control group and therefore was approved by the FDA for treating HRPC patients.

      There was a median survival benefit of 4.1 months. This means that half the patients taking Provenge survived longer than that time and half shorter. About a third of the men in the latest clinical trial were still alive 3 years after treatment! Provenge was found to be an effective treatment, coupled with a low incidence of side effects,. While nothing less than a cure is a true “breakthrough,” extending survival continues to open up new treatment methods under development to HRPC patients.

      One possibility that is sure to get additional study in the future is whether patients would benefit from a “booster” Provenge treatment at a later time. One early study seems to indicate that a Provenge booster may increase PSA doubling times in certain patients. It is fair to say that research on this possibility may be in the news in the future.

       Question: What about using Provenge with other treatments, such as before or after chemotherapy or with bisphosphonates?

      Answer: A notable percentage of men from the clinical trial (20 percent) had already received chemotherapy when they were treated with Provenge, so it would be expected that some chemotherapy patients would also qualify for Provenge. An interesting analysis from a previous clinical trial found that men seemed to have significantly prolonged their lives on chemotherapy (Taxotere or docetaxel) if they had received Provenge before the chemotherapy. Thus it seems Provenge may have improved the efficacy of a later treatment. This will certainly get more research going forward.

      In the Provenge IMPACT clinical study, almost half (48 percent) of the men were on osteoporosis drugs when the study started. Therefore, Provenge can be given in men who are or are not on these medications.

      The biggest concern may be getting chemotherapy and/or prescription steroid medications just before starting Provenge treatment. Because these medications can theoretically reduce the immune response of Provenge, patients should avoid steroid use within 28 days or chemotherapy within 3 months of starting Provenge treatment.

       Question: What if I have HRPC and there is no cancer in my bones, but there is some cancer in my lymph nodes. Would I still be able to receive Provenge?

      Answer: Since a small percentage of men with HRPC in the Provenge clinical trials had cancer only in their lymph nodes, insurance should cover some men in this situation, allowing them to receive Provenge. However, there are many men with prostate cancer who have cancer in their lymph nodes, but some are still sensitive to androgen-suppression therapy and/or radiation. These men do not have HRPC, which means they would not be eligible to receive Provenge at this time.

      Worth considering in this section are any diet or dietary supplements that would benefit patients undergoing Provenge treatment. Generally speaking, “less is more” during this treatment. In other words, Provenge patients should not take any supplement that says it is an “immune booster” or “immune modulator,” or that makes claims to “support the immune system.” Theoretically, such supplements could block or disrupt the action of Provenge, so why take that chance?

      Overall, what is good for your heart is also good for your immune system. If you think about all the things in diet and life that are heart healthy, almost all of them turn out to be immune-healthy recommendations, or help make your immune system as healthy as possible. Check the section later in the book for more information on heart-healthy diet and supplements, but here are a few quick suggestions pertaining to your immune system:

      Tip 1: Alcohol Moderate to slightly higher-than-moderate levels of alcohol intake from any source (beer, hard liquor, or wine) on a regular basis can reduce the effectiveness of your immune system.

      Tip 2: Blood Pressure and Cholesterol High blood pressure (hypertension) that is not under control and high cholesterol can lead to immune problems and make heart disease worse.

      Tip 3: Blood Sugar High glucose (sugar) and other abnormal health numbers (hemoglobin A1c) could compromise your immune system.

      Tip 4: Dental Disease An unhealthy mouth can cause bacteria and other organisms to travel to other body sites, stressing the immune system.

      Tip 5: Diet Heart-healthy diets (such as the Mediterranean diet, for example) are immune healthy, but this also implies that heart-unhealthy diets are immune-unhealthy diets.

      Tip 6: Environment Regular exposure to some pollutants, such as lead and exhaust fumes, can reduce your immune function.

      Tip 7: Medications Some pills or injections (for severe arthritis, for example, or for autoimmune disease, or even for severe allergies) work by reducing the effectiveness of your immune system, so review with your doctor any medications that might inhibit the impact of Provenge. Note: Diphenhydramine (Benadryl) is given only before each Provenge infusion, but this drug has a minor, temporary, and specific immune-reducing effect (to reduce side effects of the infusion itself), so this is no problem.

      Tip 8: Exercise Moderate, regular exercise is healthy, but extreme exercise (daily triathlon training, for example) can cause immune suppression.

      Tip 9: Sleep Reducing sleep by even a few hours can cause immune suppression. Naps and regular sleeping hours should be encouraged.

      Tip 10: Tobacco Smoke causes large nutrient reductions in the blood. Immune cells rely on some of these key nutrients for activity, so stay away from smokers, smoking, and smokeless tobacco.

      Tip 11: Stress Large quantities of immune-suppressing hormones are released with stress, so try to monitor your stress levels. If you need help with your stress levels, talk to your doctor or caregiver about this.

      Tip 12: Vaccinations Ask your doctor whether you are up to date with your vaccinations, and which ones (if any) you qualify for at this time. For example, research has shown that if you qualify for a flu shot and get it, it may provide other less obvious benefits for you. The shot may reduce your risk of other diseases (cardiovascular) and infections while your immune system is being challenged by other things.

      Tips 13 and 14: Weight and Waist Higher body weight, body mass index (BMI), and especially belly fat increases over a short period of time can overstimulate the immune system and direct it toward the fat tissue as opposed to more important things, such as an infection or another condition.

       Question: What about vitamin D supplements? I hear they are good for the immune system.
